1. Personal appearance.
2. Passport with no less than 3 months validity over the visa expiration;
3. Accomplished application form with 2 pcs photos 1.77 x 1.37 inches colored with white background studio taken;
4. Original and a photocopy of all documents required for each applicant.
5. Certificate of employment (to include job description) or Business registration certificate and financial statement.
6. Guarantee letter from the Company.
7. Bank certification of the applicants dollar and peso accounts, passbook, statement of account. If available, travelers checks and international credit cards.
8. Declaration of the applicants annual income (ITR).
9. Confirmed round trip ticket upon release.
10. Itinerary, confirmed round trip flight booking and hotel vouchers or hotel booking under the name of the traveler.
11. Corporate ITR and/or Company bank certifications.
12. Business Registration Certificate and Financial Statement.
13. Invitation (not more than 90 days from the date of application in the Embassy) from the Italian Company indicating the name of the applicant, the purpose and the duration of stay in Italy. Furthermore, it should contain a declaration that the signatory guarantees the applicant and is aware of the penal responsibilities he assumes whenever the invitee violates the immigration law (Art.12, comma 1 of Dlgs 286/98 as amended by Law 189/02). It should also indicate the expenses to be shouldered when the applicant (or his employer) does not guarantee the said expenses.The signature of the representative of the Italian company that made the invitation should be authenticated, or accompanied by a copy of a document of identity of the signatory.
14. Original certification from the authorized Chamber of Commerce for the Italian Company (issued not more than 90 days from the date the document is presented to the Embassy).
15. Travel insurance with a coverage of at least 30,000 euros. (Travel dates + additional 15 days)

• Business Financial Statement
• Mayor’s Permit

7. Bank certification for dollar and peso accounts accompanied by their corresponding passbook and or 6 months bank statements of account. If available, travelers checks and international credit cards.
8. Declaration of annual income (ITR).
9. PRC ID copy (if applicable)
10. Confirmed round trip ticket upon release.
11. Itinerary, confirmed round trip flight booking and hotel vouchers or hotel booking under the name of the traveler.
12. A letter of invitation addressed to the Embassy, (format of this letter may be secured at the Italian Embassy or downloaded from www.ambmanila.esteri.it), in case the traveler will stay in a friends or relatives residence. The invitation may be made by an Italian national or a foreign resident legally staying in Italy. A document of identity or permit of stay should accompany the letter of invitation.
13. For all minors, an Affidavit of Support and Consent from both parents.  In addition, a DSWD clearance for minors not traveling with parents, School Certificate, copy of School ID and School Calendar.
14. Travel insurance.
