NOTE: The embassy entry is limited to applicants only so an on-site liaison officer is not available.

1. Original passport valid for 6 months above including previous passport/s
2. DS 160 form (visa application form) – filled out online and printed along with the confirmation sheet (website: https://ceac.state.gov/genniv/)
3. Visa fee payment receipt validated at RCBC (payable in peso only)
4. 3 pcs. 2 x 2 inches photos colored with white background studio taken
5. Copy of the invitation letter
6. Original guarantee letter
7. Original employment certificate with position, salary and tenure
8. Business registration – if owning a business (either form the Dept of Trade & Industry-DTI, Securities and Exchange Commission-SEC, and/or Municipal Permit)
9. Bank statement/s within the past 3 months and bank book
10. Credit card/s statement within the past 3 months
11. Income Tax Return (ITR)
12. Birth certificate and marriage contract issued by the National Statistics Office (NSO) if accompanied by family members
13. Proof of other financial assets
14. Personal appearance by appointment
15. Photocopy of passport
16. Photocopy of DS 160 confirmation sheet
