1. Original passport valid for 6 months above including previous passport/s
2. Duly filled-out and completed visa application form on A4 size paper with no erasures and personal handwriting only
3. 2 pcs photos – passport size colored with white background recently studio taken and thumb size face with no jewelry/headband/caps and no dentures appearing do not staple photos
4. Daily schedule in Japan on A4 size paper
5. Flight itinerary on MDTI printout and use A4 size paper (For company incentive trip)
6. If employed – original employment certificate with position, salary and tenure on A4 size paper If owning a business – copy of business registrations municipal and national on A4 size paper
7.   Income Tax Return (ITR) – should be signed by both applicant and his employer. Photocopy only on A4 size paper
8. Current year original PSA birth certificate – if no previously used Japan Visa

• If the certificate is unreadable, submit Birth certificate issued by Local Civil Registrar.
• If the birth certificate is “LATE REGISTRATION”, submit Baptismal Certificate and School Record (Form 137).
• If there is no record of birth in PSA, submit Birth Certificate issued by Local Civil Registrar and Negative Certificate issued by PSA.

9. Current year original PSA marriage contract – if no previously used Japan Visa

• If the certificate is unreadable, submit Marriage certificate issued by Local Civil Registrar.
• If there is no record of marriage in PSA, submit Marriage Certificate issued by Local Civil Registrar and Negative Certificate issued by PSA.

10. Hotel reservation on A4 size paper (For company incentive trip)
11. Original personal bank certificate/s with 3 months bank statements
12. Authorization letter for MDTI to process and release visa on A4 size paper – see under visa forms Japan folder
13. In case a representative will file for the business visa application, an authorization letter from the applicant is required on A4 size paper
14. School registration copy &/or certificate & ID copy on A4 size paper – if a student
15. PHL ACR I-Card (for foreign nationals) – photocopy only on A4 size paper
16. for support letter – use A4 size letter and see sample on visa forms under Japan folder

• Proof of relationship between applicant and guarantor (PSA Birth Certificate) – required even if with previously used Japan Visa
• Bank Certificate with 3 months Bank Statements
• Photocopy of Income Tax Return – should be signed by applicant and his employer
• Employment Certificate
